A successful transition process includes families of students with disabilities. Families can support the transition from high school to adulthood by learning how to assess their teen’s strengths, interests and needs, help to establish measurable goals for the future, identify appropriate transition services and work with the Committee on Special Education (CSE). Participants will gain information on the district responsibilities in the transition planning process, be provided resources, tools and activities to support their students as well as learn how to be a key stakeholder in the process.
